Whistleblowing Line Application Form

This notification is made due to legal obligation pursuant to Article 10 of the “Personal Data Protection Law” No. 6698 (briefly PDPL). Under the PDPL, any information pertaining to an identified or identifiable natural person is defined as personal data and our Bank carries out its activities with respect to processing of personal data by taking necessary security measures to protect fundamental rights and freedoms, particularly the right to privacy. Our aim is to provide you with information regarding the processing purposes of your personal data, to whom and for what purposes your processed personal data may be transferred, the methods and legal basis for collecting your personal data, as well as your rights related to your personal data as regulated in Article 11 of the PDPL. Identity of the Data Controller and of its representative, if any: Will be processed by Albaraka Türk Katılım Bankası A.Ş. (“Albaraka Türk”) resident at İnkılap Mahallesi Dr. Adnan Büyükdeniz Cad. No:6 Ümraniye/İSTANBUL. Purpose of processing of personal data: Your Full Name, Mobile Phone Number, and E-mail address will be processed solely for the purpose of contacting you regarding the matter you submitted through the Whistleblowing Application Form available on Albaraka Türk’s website (İhbar Hattı Başvuru Formu - Albaraka Türk) or via the Whistleblowing Line. Method and legal basis of collection of personal data: Your personal data is collected automatically from you, based on legal basis of your explicit consent as specified in subparagraph (a) of paragraph 2 of Article 5 of the Personal Data Protection Law. To whom and for which purposes the processed personal data may be transferred: The personal data mentioned above is not transferred to any third parties. Other rights referred to in Article 11 of the PDPL: Without prejudice to the all relevant exceptions, especially the cases provided for in Article 28 of the PDPL, you can transmit your requests under the Article 11 of the PDPL regulating the “rights of the data subject”; In accordance with the “Communique on the Procedures and Principles for Application to the Data Controller”, to the Bank's “albarakaturk@hs03.kep.tr” REM address or through the Branches of our Bank in writing and signed, Through a notary to İnkılap Mahallesi Dr. Adnan Büyükdeniz Cad. No:6 Ümraniye/İSTANBUL address, To the e-mail address kvkk@albarakaturk.com.tr by using your e-mail address registered in our Bank. The application must include; Your first name, last name, and, in the case of a written application, your signature, If you are a citizen of Turkish Republic, your Turkish ID number, and if you are not a citizen of Turkish Republic, your nationality, passport number, or if available, your ID number, Your address of residence or workplace for notification purposes, If available, your registered and verified email address, phone number, and fax number previously provided to our Bank for notification purposes, and Subject of your request. I hereby acknowledge and declare that all personal data belonging to me, held by Albaraka Türk Participation Bank A.Ş., is subject to the regulations stipulated by the Personal Data Protection Law No. 6698 (briefly PDPL) and other relevant legislation, and and that the Bank has fully and clearly informed/noticed me regarding my rights under these laws and regulations through the 'Whistleblowing Line Information Notice on PDPL'.

I hereby give my explicit consent for the processing of my Full Name, Mobile Phone Number, and E-mail address in order for your bank to contact me regarding the matter I have submitted via the Whistleblowing Line Application Form.

I approve that your Bank can send commercial electronic messages to me via telephone, call center, fax, automatic call machines, e-mail, short messages and any other means for goods, products, services, campaigns, advertisements, marketing and other matters belonging to your Bank or your commercial business partners, affiliates or third parties. I acknowledge and declare that the bank has provided me with full, clear, and understandable information/clarification regarding my consent and my rights within the scope of Personal Data Protection Law No. 6698 and other relevant legislation.
